Normally caused by scored disc on the siggy. The pads are poor quality on certain ages cars and can crumble and score the disc. New pads tend not to fix the issue. New discs may. Take it to get checked. What year and how many miles are on the clock. Another option could be a sticking brake caliper.

If your discs were warped the steering wheel would vibrate under braking.
It's probably either pads worn down (although this should trigger a warning in the dic) dirt trapped between the pad and disc or, most probably, needing some copperslip appplied to the back of the pad and shim.

The pads don't have sensors so no warnings in the DIC. They have an audible wear bar.
